Use day-old bread
Stale bread soaks up custard without turning to mush; if yours is fresh, cube it and dry it in a 300°F oven for 10 minutes.
Soak it overnight
Cover and refrigerate at least 4 hours, or overnight, so the egg and milk reach the center of every cube.
Cover then uncover
Bake covered with foil for the first 25 minutes, then uncover for 20 to 25 more so the top browns without drying out.
Let it rest
Give the casserole 10 minutes out of the oven before scooping so the custard sets and squares hold together.
